Described is a medicament injection device (100) comprising a housing (102) containing a syringe (300), a pain reduction device (200) coupled to the housing (102), a coupling mechanism (110) disposed in the housing (102) and operably coupled to the pain reduction device (200), and a drive mechanism (106) disposed in the housing (102) and operably coupled to the coupling mechanism (110). Activation of the drive mechanism (106) actuates the coupling mechanism (110) to activate the pain reduction device (200), which either releases a pain reduction agent or alternatively cools or heats the skin region adjacent the injection site.
