The use of a cannabidiol (CBD) preparation for the treatment of seizures associated with rare epilepsy syndromes, in particular perisylvian fissure syndrome. The seizures associated with perisylvian fissure syndrome include focal seizures with impairment. The CBD preparation may comprise 95% (w/w) CBD and not more than 0.15% (w/w) tetrahydrocannabinol (THC). Preferably the CBD preparation comprises greater than or equal to 98% (w/w) CBD and less than or equal to 2% (w/w) other cannabinoids, which may comprise THC, cannabidiol-C1 (CBC-C1), cannabidivarin (CBDV), and cannabidiol-C4 (CBD-C4), where the THC may be a mixture of trans-THC and cis-THC. The CBD preparation may be used in combination with one or more concomitant anti-epileptic drugs (AEDs) such as levetiracetam, clonazepam, diazepam, and lacosamide. The CBD in the CBD preparation may be synthetic or isolated from cannabis plant material. Preferably the dose of CBD is between 5 mg/kg/day to 50 mg/kg/day.
