An inhaler comprising a housing to receive a strip having a plurality of blisters, a blister drive member 57 which rotates about a first axis, a mouthpiece 202 mounted to the housing and a cap 201 that covers the mouthpiece 202 in a closed position and rotates about a second axis C. The first and second axes are parallel and spaced apart. Rotation of the cap 201 to uncover the mouthpiece 202 causes the blister drive member 57 to rotate an unopened blister into alignment with the mouthpiece 202 and then actuates a blister piercing member 210 to puncture the blister, releasing the contents. A drive gear 16 may couple the cap 201 and blister drive member 57 during at least part of the cap motion.
