A mechanism for use with a syringe to permit repeated injections of variable doses has a body (10) in which is slidably mounted a syringe piston operating rod (12). A plunger (11) surrounds part of the rod (12) and is slidably mounted in the body (10), a releasable screw-threaded connection (15, 30) being formed between the plunger and the body. An automatic one-way ball clutch (20) inteconnects the plunger (11) and rod (12), and a further one-way ball clutch (32, 36) interconnects the body (10) and rod (12). A dose is dispensed by rotating plunger (11) to thread that plunger away from clutch (32, 36) until a required dose is displayed through window (42), the rod (12) being held stationary by clutch (32, 36). Then on driving the plunger forwadly, the clutch (20) connects the plunger to rod (12), so that the rod (12) is also driven to project further from the body (10). Movement is limited by the forward end of the plunger striking cup member (32) secured in the body.
