A cephem compound of the formula : wherein R 1 is amino or a protected amino, R 2 is lower alkyl which may have one or more suitable substituent(s), R 3 is COO e , carboxy or a protected carboxy, R 4 is hydroxy(lower)alkyl or protected hydroxy(lower)alkyl, R 5 is amino or a protected amino, R 6 is hydrogen or lower alkyl, X 9 is an anion, and n is 0 or 1, with proviso that (i) when R 3 is COO e , then n is 0, and (ii) when R 3 is carboxy or a protected carboxy, then n is 1, and pharmaceutically acceptable salt thereof, processes for their preparation and pharmaceutical compositions comprising them as active ingredient in admixture with pharmaceutically acceptable carriers.
