Mechanism of action
Hydrochlorothiazide is a thiazide diuretic that blocks the sodium-chloride cotransporter in the kidney's distal convoluted tubule, promoting sodium and water excretion and reducing blood volume and vascular resistance. However, thiazides increase urinary potassium loss, which can lead to hypokalemia; the addition of potassium supplementation maintains normal serum potassium levels and prevents cardiac arrhythmias and muscle weakness associated with potassium depletion.
