Mechanism of action
Phenylephrine selectively binds to alpha-1 adrenergic receptors on vascular smooth muscle, causing vasoconstriction and increased systemic vascular resistance. This elevation in afterload can initially increase blood pressure but may decrease cardiac output due to the increased workload on the heart, or maintain it depending on compensatory mechanisms and baseline cardiac function.
